Dynamic fields for tickets

I would like the fields where contact/ticket information is linked to be dynamic, so that when speaking with someone in the healthcare sector who wants to schedule or purchase a service for multiple people, I can dynamically add fields to the conversation and establish relationships between the tickets. This would help make the conversation less rigid.
